Hourly Salary: $57.11 – $60.65
The Attending Nurse Practitioner (NP) reports to the Director of Care and is responsible for and is also accountable to the Medical Director for meeting the Long-Term Care Home’s (LTCH) policies, procedures, and protocols for medical services. The Attending NP works within their legislative scope of practice as described and outlined by the College of Nurses of Ontario.The Attending NP collaborates with the resident and family/caregiver, and the health care team in the development, implementation and evaluation of the resident’s plan of care; provides leadership, education and mentorship to LTCH staff.
– Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice
– Post-graduate education and experience in gerontology
– Specialty certification in gerontological nursing
– Post-graduate certificate and experience in palliative care
– CNA Certificate in Geriatric Nursing
– Membership in professional association e.g. NPAO
– Advanced clinical experience, skills and judgement in geriatric medicine and mental health
– Superior ability to communicate effectively and concisely, both orally and in writing and through a variety of means
– Strong knowledge of community and regional services
– Ability to function and contribute as an effective team member including providing mentoring, education and coaching within the team
– Ability to work independently with the capacity to case review and consult with physicians
– Strong leadership skills and demonstrated ability to organize, prioritize and multitask efficiently
– Excellent computer, presentation and educational skills
– Strong interpersonal skills to work with diverse client groups with varying levels of comprehension and language capability
– Effective listening, observation and facilitation skills to effectively assess the client and provide appropriate treatment and consultation
– Ability to use independent judgment and maintain confidentiality of records, actions, and information
CONDITIONS OF EMPLOYMENT:
– Maintain certifications
– Ensure appropriate Professional Liability Protection is available according to the requirements of the CNO
– May be required to travel to various work locations and provide own reliable transportation
This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
Offers of employment made to external candidates or internal candidates transferring into Long-Term Care are conditional upon providing proof of COVID-19 vaccination as per the SJCG Immunization Policy IMS-04-006 or proof of valid medical exemption, prior to the offered effective date.
