The Transitional Discharge Worker is primarily responsible for ensuring seamless, safe and supportive transition of individuals from inpatient medical care to community home settings. The Transitional Discharge Worker is responsible for receiving clients from the care of professional contracted staff and linking them to community-based mental health and addiction and other personal support services to ensure the effective transfer of client information and recommendations to optimize successful community re-integration.

Qualifications and Requirements: A Social Work Degree is preferred. A three year Diploma in the Social Services field or a degree in another field may be considered if complimented by relevant work experience. Three (3) years direct experience in the delivery and/or coordination of transitional care services; demonstrate sound knowledge and experience in community-based care service delivery; candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered.

Hours: 32.5 per week
