Position Overview:
The Community and Personal Support Worker is a multi-functional para-professional who is responsible for the delivery of in home support and personal care to ill, frail, and persons with disabilities of all ages so that they may remain independent in their own homes and communities for as long as possible. With supervision provided by Dilico’s Community and Personal Support Services Managers, the Community and Personal Support Worker collaborates with the Home and Community Care Registered Nurse and other Health staff, to ensure the provision of culturally appropriate support and assistance to eligible clients including provision of support with activities of daily living, personal care, caregiver relief, palliative care, speech and language pathology support, adult life enrichment program services and basic foot care. If not located at identified office, can travel from another office. Dilico provides a vehicle rental and accommodations with meal reimbursement.
Qualifications and Requirements:
- A minimum one-year PSW Certificate is required
- A minimum of two (2) years direct experience in the provision of in-home personal care and/or community based home care service delivery
- Experience in the facilitation and/or coordination of community programs and services is an asset
- Candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered
- Must have a Personal support Worker Certificate or be willing and able to complete Personal Support Worker Certificate training
- Be willing to complete speech language training as required
- Must demonstrate the willingness and ability to work with clients of all ages, including infants, children, adults and the elderly
- Must possess a Standard First Aid, BLS, and CPR Certificate or be willing to obtain one
- Must be able to work flexible hours including days, evenings, and weekends; in a home, school, and/or community setting as required
- Must be able to travel to all Dilico communities on a weekly basis (preference given to candidates living in the primary service area
- Must have a valid Ontario G driver’s license and access to a personal vehicle
- A Criminal Record and Vulnerable Sector Search check is required upon hiring
